Participating Insurances

We are in network with Horizon BCBS, Cigna and United/Oxford.  The office manager will collect your information and verify benefits prior to your first appointment. Fees for sessions are $100-$125, with limited sliding scale available.

Please note, you have the right to a Good Faith Estimate if you choose not to utilize insurance.  Forward Progress will provide this in compliance with the No Suprises Act to prevent surprise medical expenses.
